First Call

Care-plan coordinator Elise Moreau reviews your inquiry, listens to your current situation, and calls you back within one working day. During this introductory phone conversation, we discuss your immediate clinical needs, clarify our 8 km service area, and schedule your initial assessment visit at a convenient date and time.

Kitchen-Table Assessment
Next, an experienced nurse conducts a 60-minute assessment visit in your home for $60. We sit down together at your kitchen table to evaluate your medical history, current prescriptions, daily physical mobility, and domestic environment. Family members or caregivers are warmly invited to participate. This unhurried conversation allows us to understand your personal routines and specific healthcare preferences before drafting your care recommendations.

Written Plan Agreed
Within three days following your home assessment, we deliver a comprehensive written care plan alongside a transparent cost estimate. The document specifies visit durations, frequency, assigned clinicians, and defined health goals. Once you review and sign the agreement, we immediately share a copy with your attending doctor to ensure complete medical alignment before your regular home nursing or therapy visits begin.

Review Every 30 Days
Healthcare needs evolve over time, which is why we conduct a formal care-plan review every 30 days. Your nurse or therapist assesses your clinical progress, evaluates recovery milestones, and listens to feedback from you and your family. If dressing frequency can be reduced or rehabilitation goals have advanced, we adjust the visit schedule and fees accordingly, keeping your medical team updated throughout.

It also lists emergency escalation paths, contact details for Elise Moreau and our clinical team, and a plain itemization of visit fees so that financial expectations remain clear from day one.
